COVID Safe Check-in
As directed by the current NSW Public Health Orders, it is mandatory to complete a COVID Safe Check-in when entering Willoughby Leisure Centre.
All adults (16+ years old) are also required to be fully vaccinated to enter Willoughby Leisure Centre until 1 December, 2021. Proof of vaccination certificate or a valid COVID-19 vaccine medical contraindication form (as per NSW Health) is required at time of entry.
Please note, we are required by NSW Public Health Order to collect your details upon entry using the Service NSW COVID Safe Check-in system. Entry to Willoughby Leisure Centre may be denied if the Service NSW COVID Safe Check-in is not completed or without valid proof of full COVID-19 vaccination.
How can I check in?
There are multiple options available to complete your COVID Safe Check-in.
1. Use the Service NSW facility QR code

- if the Service NSW app is already installed on your phone, this app will open and prompt you to confirm your contact details via the COVID Safe Check-in tool.
- if the Service NSW app is not installed on your phone, you will be directed to a webpage where you will be given the choice to download the Service NSW app to check in or check in using the Service NSW webform for Willoughby Leisure Centre.
